For individuals with Autism, other disabilities, their families and caregivers.

What to expect:
-Flu Vax for ages 3+
-Covid Vax for ages 12+
-Sensory-friendly (quiet, low-light)
-Registration encouraged but not required
-Arrive no later than 5:45pm in order to receive a vaccination
-Assistance for Spanish speakers
-The Nonprofit Collaborative is ADA compliant

Gift cards for individuals who complete the brief follow-up survey

What to bring:
1. Completed Giant Pharmacy consent form (attached to confirmation email)
2. Insurance card

No health insurance? No problem!
AUSOM will provide funding for the uninsured. Please indicate this as part of your registration or notify the clinic staff upon arrival.

Pre-registration is encouraged. Enter a “Quantity” for the total number of people receiving a vaccination.
